Getting Ready

Please read ALL the instructions carefully before collection. Results might be affected if not collected properly.

Note

On Requisition Form: Please clearly print your Name, Date of Birth, Gender, & Collection Date.

Also include Physician’s Name & tests requested.

The following might cause sample rejection:
a
Missing or incomplete information
b
Nicknames or abbreviated names
c
Mismatch in information between Requisition Form & Collection Strip

Precautions

Avoid collection during menstruation or urinary tract infection.
2
Avoid excessive fluid intake the evening prior to urine collection. An overly dilute urine specimen may be rejected.

Supplements and medications may impact results. Please consult with your healthcare provider.

Keep out of reach of children.

Environment

For Environmental Pollutants Testing only:

Preservatives that need to be avoided for 48 hours before collection include:
a
Sorbic Acid/Sorbate (preservative) (E200-203)
b
Benzoic Acid/Sodium Benzoate (preservative) (E 210-213)

Diet Restrictions

Food suggested to be avoided for 48 hours prior to collection:

Beverages: Tea, Coffee, Alcoholic Beverages, Juice

Vegetables/Fruits: Avocado, Kiwi, Tomato, Pineapple, Banana, Plantain, Grapes/ Raisins, Plums/Prunes, Seed/Nuts, Walnut, Pecan

Others: Ketchup, Vanilla Extract, Jell-O™, Aged and Processed Cheeses
